Once you determine we are the best fit for you, we will send you a new patient intake packet. You can fax, scan/email, or drop your packet off to our Los Gatos location.

We use this information as a part of our comprehensive assessment. Following your assessment we will provide you with an individualized, integrative treatment plan.
